A new patient with no apparent disability comes to your office with an unleashed but behaved dog. Your receptionist tells the person that dogs are not permitted in the office, but the person barks back "It's a service dog and I know my rights so you can't hassle me about it."

CAP's Risk Management and Patient Safety Department offers the following guidelines to help physicians who are retiring or otherwise closing or selling their practice, or for those handling the estate of a deceased physician.

When documenting patient encounters, CAP physicians are encouraged to review their notes prior to signing. If there is a need to correct, modify, or change the record, you should identify the note, enter the new data, and/or refer to the specific encounter, if necessary.

Introducing an electronic medical record system into a medical practice enables physicians and their staff to provide more efficient care by making patient information more accessible. It also presents some risks.
