Group Disability and Term Life Program

CAP Group Disability

• The monthly benefit of up to $2,000 is yours as a CAP member. You do not need to do anything to receive this benefit.
• If you are interested in the additional monthly benefit of $4,000 (for a total amount of coverage of $6,000), please complete this Quote Request Form. 
• The waiting period is 90 days. (disability only)
• Specialty-specific definition of disability (disability only)
• Benefits payable until age 65 (disability only) Note: after age 65, the benefits incrementally decrease till age 70 and from there forward is a one-year benefit.

If you currently have an individual disability policy, this is a great way to add coverage to, but not necessarily replace, your existing policy.

• The death benefit coverage of $20,000 is yours as a CAP member. You do not need to do anything to receive this benefit.
• Designated Beneficiary Form

Family Term Life Benefit

• $30,000 Spouse of Member
• $10,000 per Child(ren) of Member
